Before Your Visit

It is recommended that prior to your office visit you should contact your insurance carrier to verify your benefits and coverage. Any copays required by your insurance company must be paid on the date of service.  Effective January 1, 2012, all deductibles applicable to procedures are required in advance.

Here are some questions to ask your insurance company:

  1. What is my copay to see a specialist?
  2. How much is my deductible?  How much has already been met?
  3. What are my colonoscopy screening benefits?
  4. Does a [colonoscopy/CT/EGD/ERCP/other applicable procedure] require a preauthorization?

Financial Policy

A summary of our financial policy is as follows:

  1. Copays and deductibles are payable at the time of service (or prior to any procedures).
  2. Patients must read and sign informational notices, etc., prior to being seen.
  3. Patients must be able to provide proof of insurance at the time of service.
  4. Any coinsurance amounts not paid at the time of service must be paid promptly.
  5. Self pay pricing is available but must be paid at the time of service (or prior to the scheduling of procedures).
  6. Our office does not bill Medicaid retroactively–self pay patients who make arrangements for self pay pricing will be accepted as such.
  7. There is a $35 charge for any forms our office is asked to fill out.
  8. Patients who repeatedly miss appointments may be assessed a $50 Late Fee, payable prior to being seen again.
  9. Bills incurred as part of your treatment regimen are the patient’s responsibility.  Precertification is obtained for procedures we schedule, but it is not a guarantee of payment.  Balances not paid by the insurance company are the responsibility of the patient.
  10. Procedures will be billed according to coding and billing guidelines.  100% screening  colonoscopy benefits only apply under certain circumstances.  Please read the financial policy in order to obtain a better understanding of when the 100% benefit applies.
